RECOMMENDATION(S):

Correct the Board Order which was approved by the Board of Supervisors on September 27, 2011, (C.43) with Contra Costa Rheumatology, LLC to reflect the intent of the parties in which the legal name should have read Mt. Diablo Rheumatology Consultant, Inc., a Corporation, in an amount not to exceed $200,000, to provide professional rheumatology services at Contra Costa Regional Medical Center and Contra Costa Health Centers, for the period from September 1, 2011 through August 31, 2012.

FISCAL IMPACT:

This Contract is funded 100% by Enterprise Fund I.

BACKGROUND:

On September 27, 2011, the Board of Supervisors approved Contract #26-704 with Contra Costa Rheumatology, LLC, for the period from September 1, 2011 through August 31, 2012, for the provision of professional rheumatology services at Contra Costa Regional Medical Center and Contra Costa Health Centers including clinic session, consultation, training and/or medical and surgical procedures.

BACKGROUND: (CONT'D)

Due to the mutual mistake of the parties, the Contract did not accurately reflect the correct business name of the Contractor. The Contractor’s official business name should have read Mt. Diablo Rheumatology Consultant, Inc.

Approval of this Board Order will allow Mt. Diablo Rheumatology Consultant, Inc. to provide professional rheumatology services at Contra Costa Regional Medical Center and Contra Costa Health Centers including clinic sessions, consultation, training and/or medical and surgical procedures, through August 31, 2012.

CONSEQUENCE OF NEGATIVE ACTION:

If this correction is not approved, the Department would not be able to process the Contract under the correct business name of Mt. Diablo Rheumatology Consultant, Inc.
