BACKGROUND:

The Public Health Division was recently awarded funding from the State Department of Public Health in order to implement the Nurse Family Partnership program within Contra Costa County. The programs objective is to provide comprehensive and coordinated health care related services to improve outcomes for families residing in identified at-risk communities. These five positions are necessary in order for the Public Health Division to effectively implement and maintain the Program.

If this action is not approved, the Department will not be able to meet the mandates set forth in the grant which in turn will result in the Department not being able to provide much needed health care related services to those residents in at-risk communities within Contra Costa County who qualify for this program.
