RECOMMENDATION(S):

APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Contract #77-430-2 with Jiva Health, Inc., a corporation, in an amount not to exceed $2,000,000, to provide endocrine, diabetes, allergy and sleep medicine services for Contra Costa Health Plan (CCHP) members and County recipients for the period January 1, 2023 through December 31, 2024.

FISCAL IMPACT:

This contract will result in contractual service expenditures of up to $2,000,000 over a two-year period and will be funded 100% by CCHP Enterprise Fund II revenues.

BACKGROUND:

CCHP has an obligation to provide certain specialized medical specialty health care services for its members under the terms of their Individual and Group Health Plan membership contracts with the county. Including but not limited to: endocrine, diabetes allergy and sleep medicine services. This contractor is part of the CCHP Provider Network providing these specialty medical services since January 1, 2022.

BACKGROUND: (CONT'D)

On January 18, 2022, the Board of Supervisors approved Contract #77-430 with Jiva Health, Inc., in an amount not to exceed $2,000,000, for the provision of endocrine, diabetes, and allergy specialty services for CCHP members and county recipients the period January 1, 2022 through December 31, 2022.

On April 12, 2022, the Board of Supervisors approved Contract Amendment Agreement #77-430-1 with Jiva Health, Inc., effective March 1, 2022, to increase the payment limit by $25,000, from $2,000,000 to a new payment limit of #2,025,000, for the provision of additional endocrine, diabetes, allergy specialty and sleep medicine services for CCHP members and County recipients, with no change in the original term of January 1, 2022 through December 31, 2022.

Approval of Contract #77-430-2 will allow the contractor to continue providing these medical specialty services through December 31, 2024.

CONSEQUENCE OF NEGATIVE ACTION:

If this contract is not approved, certain specialized endocrine, diabetes, allergy, and sleep medicine specialty services for CCHP members under the terms of their Individual and Group Health Plan membership contracts with the County will not be provided.
