RECOMMENDATION(S):

APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Contract #77-255-1 with Suncrest Hospice San Jose, LLC, a limited liability company, in an amount not to exceed $900,000, to provide hospice services for Contra Costa Health Plan (CCHP) members, for the period December 1, 2020 through November 30, 2023.

FISCAL IMPACT:

This contract will be funded 100% by CCHP Enterprise Fund II.

BACKGROUND:

On October 8, 2019 the Board of Supervisors approved Contract #77-255 with Suncrest Hospice San Jose, LLC for the provision of hospice services for CCHP members for the period December 1, 2019 through November 30, 2020.

Approval of Contract #77-255-1 will allow the Contractor to provide hospice services to CCHP members through November 30, 2023.

CONSEQUENCE OF NEGATIVE ACTION:

If this contract is not approved, certain specialized health care services for CCHP members will not be provided.
