BACKGROUND:
The Health Services Department is seeking funding for the County’s Coordinated Outreach, Referral and Engagement (CORE) program. CORE teams locate and engage homeless individuals and families throughout Contra Costa County. The program serves as an entry point into the County’s coordinated entry system for unsheltered persons and works to locate, engage, stabilize and house chronically homeless individuals and families.
