RECOMMENDATION(S):

APPROVE and AUTHORIZE the Health Services Director, or designee, to transmit an application along with the necessary certification and assurances, to the City of Concord for Concord/Pleasant Hill Health Care District (CPHHCD) and accept funding, in an amount not to exceed $13,000, for homeless outreach services provided by the Coordinated Outreach, Referral and Engagement (CORE) program for the period July 1, 2023 through June 30, 2026.

FISCAL IMPACT:

The funds requested of the City of Concord, CPHHCD are essential and will provide up to $13,000 of program funding for homeless outreach services. No County match is required.

BACKGROUND:

The Health Services Department is seeking funding for the County’s Coordinated Outreach, Referral and Engagement (CORE) program. CORE teams locate and engage homeless individuals and families throughout Contra Costa County. The program serves as an entry point into the County’s coordinated entry system for unsheltered persons and works to locate, engage, stabilize and house chronically homeless individuals and families. Approval will allow the County to receive CPHHCD funding to support the County’s CORE program.



CONSEQUENCE OF NEGATIVE ACTION:

If not approved, the Health Services Department will not receive the additional funds to support the CORE program and may have to operate at a reduced capacity.
