RECOMMENDATION(S):

ACCEPT cancellation notice from Evan H. Goulding, M.D., a self-employed individual, terminating Contract #74-401, for the provision of professional outpatient psychiatric services, effective at the close of business on December 31, 2010.

FISCAL IMPACT:

None, This Contract was funded 100% by Mental Health Realignment, offset by third-party billing.

BACKGROUND:

On September 14, 2010, the Board of Supervisors approved Contract #74-401 with Evan H. Goulding, M.D. in an amount not to exceed $104,000, to provide outpatient psychiatric services, for the period September 1, 2010 through August 31, 2011.

BACKGROUND: (CONT'D)

Evan H. Goulding, M.D., notified the Department that he wished to terminate Contract #74-401 with the County. The purpose of this Board Order is to advise the Board of Supervisors that the Department and the Contractor have agreed to terminate the above mentioned Contract effective December 31, 2010.

CONSEQUENCE OF NEGATIVE ACTION:
