Acceptance of this Award will result in a total of $285,845 from the California Department of Public Health, Tuberculosis Control Branch, for fiscal year 2014-2015. No County match required
The Contra Costa County, Public Health Department maintains a TB Control Program, which serves all reported TB patients and their contacts in Contra Costa County. Nurse case management services are provided to all people with TB to ensure they finish treatment. Outreach services are provided to reach the “Hard-to Reach” people with TB and those at high risk. The TB control staff work within the Communicable Disease Section in collaboration with the HIV/AIDS Program, Healthcare for the Homeless, Contra Costa Regional Medical Center and Health Centers, and providers throughout the County.
Acceptance of this Award will allow the Department to: (1) continue to perform prevention and control activities, and (2) improve completion of appropriate therapy which is essential to decrease TB transmission, prevent the development of drug resistance, and cure TB patients, through June 30, 2015.
The total Award $285,845 includes a base grant of $269,246, and an additional allotment of $16,599 to be used for food, shelter, incentives and enablers (FSIE) for the TB patients. The FSIE allotment will be used to purchase the following gift cards:
400 SaveMart Gift Cards with a value of $20/card for a total of $8,000.
400 Target Gift Cards with a value of $20/card for a total of $8,000.
The remaining FSIE balance of $599 will be used as needed for taxi vouchers and rent subsidies.
If this grant is not approved, the County will not receive funds to continue identifying and treating Contra Costa County residents who have active TB and ensure that they complete appropriate therapy.
