RECOMMENDATION(S):

Approve and authorize the Health Services Director or his designee (Wendel Brunner, M.D.), to execute, on behalf of the County Grant Award Extension #28-780-1 with Kaiser Permanente Community Benefit, Diablo Area, effective January 1, 2009, to extend the term of the Grant Award from January 1, 2009 through January 1, 2010, for the Asthma Club/Asthma Wise Project.

FISCAL IMPACT:

No change in the original amount of $20,000 from Kaiser Permanente Community Benefit, Diablo Area, for the Asthma Club/Asthma Wise Project. No County funds are required.  
  







BACKGROUND:

Asthma continues to be a chronic respiratory disease of enormous public health significance and is associated with significant disability and costs, both monetarily and its impact on the quality of life. On January 8, 2008, the Board of Supervisors approved Grant Award #28-780 from Kaiser Permanente Community Benefit, Diablo Area, for the Asthma Club/Asthma Wise Project, for the period from January 1, 2008 through January 1, 2009. The Division was notified by Kaiser Permanente Community Benefit, Diablo Area of their approval to extend the Grant Award through January 1, 2010.   
  
Approval of Grant Award Extension #28-780-1 will allow the County to continue to provide elementary and High School based asthma management classes and home visits to children who have asthma in the Kaiser Diablo service area through January 1, 2010.  
  
Three sealed and certified copies of the Board should be returned to the Contracts and Grants Unit.
