RECOMMENDATION(S):

Approve and authorize the Health Services Director, or his designee (Wendel Brunner, MD), to execute on behalf of the County, Contract Amendment Agreement #22–940–4 with Public Health Foundation Enterprises, Inc. (dba PHFE Management Solutions), a non-profit corporation, effective May 15, 2009, to amend Novation Contract #22–940–3, to increase the payment limit by $3,212, from $128,621 to a new payment limit of $131,833, with no change in the original term of July 1, 2008 through June 30, 2009, and to increase the automatic extension payment limit by $803, from $32,155 to a new payment limit of $32,958, with no change in the term of the automatic extension, through September 30, 2009.

FISCAL IMPACT:

This Contract is funded 98% Federal Title III C (1) of the Older Americans Act of 1965 and 2% by Federal American Reinvestment & Recovery Act (ARRA) funds, through the Employment and Human Services Department.








BACKGROUND:

On July 22, 2008, the Board of Supervisors approved Novation Contract #22–940–4 with Public Health Foundation Enterprises, Inc. (dba PHFE Management Solutions) for the period from July 1, 2008 through June 30, 2009, which included a three-month automatic extension through September 30, 2009, for the provision of consultation, technical assistance, and support to County’s Senior Nutrition Program, including coordination and supervision of Meals on Wheels activities, delivery of meals, and producing statistical and financial reports.  
Approval of Contract Amendment Agreement #22–940–4 will allow the Contractor to provide additional services to County’s Senior Nutrition Program, through June 30, 2009.
