BACKGROUND: (CONT'D)

On October 13, 2020, the Board of Supervisors approved Contract #27-162-9 with Juan R. Sequeira, M.D., in an amount not to exceed $450,000 for the provision of primary care medical services to CCHP members, for the period December 1, 2020 through November 30, 2023.
